| Admissions and Uniform Policies |
To apply for Riverbend Christian Academy, use the online application form and submit the form electronically or download and print the application form. All forms additional forms, identification items and applicable testing results must be filled out and submitted to the RCA office.
Acceptance is official only after the following procedures have been completed:

Application form – completed, signed, and returned. Non-refundable registration fee is paid at this time.

Emergency Medical Authorization Form – completed and signed.

Copy of Birth Certificate

Certificate of Immunization (Form 1148) – current, completed and signed.

DSS Form 2900 (Preschool Only)

Copy of student's SSN
-
Copy of legal guardian's driver’s license

Diagnostic testing for students in A.C.E. Program
